Introduce a `Hook` subcommand group in clap, moving `leiter context` to `leiter hook context`. This is the first step of the CLI restructuring — future steps will move `nudge` and `session-end` into the same group. Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
