feat: honor LEITER_HOME in all runtime output#41

All agent-facing instructions, error messages, and confirmation messages now use the resolved state directory path instead of hardcoding `~/.leiter`. This ensures that when LEITER_HOME is set, the agent and user see the correct paths. SPEC.md updated to mandate no hardcoded paths in runtime output.
