For readers interested in other languages (say Ruby), should we point to INSTALL files in corresponding subdirectories ?

I'd rather have all instructions for every languages in the same file, at least in the TL;DR section on top, so we don't make the other languages second-grades citizens of grpc. But it'd be up to the other maintainers of these languages to add a TL;DR blurb there.

This patch adds a static bootstrap proto that is expected to be provided
on the filesystem or command-line. This should enable Envoy to then
either fetch the rest of config from disk or reach out to the various
management servers for the rest of the APIs.

Fixes grpc#93.
