Hello, can we please add a known issue for the index templates form in Index Management UI? Related issue: #203553

It could be something along these lines:
The index templates form in Index Management UI doesn't allow creating an index template if the "Create data stream" toggle is switched off. The workaround for this scenario is to copy the request from the "Review" step in the form and run it in Console.

cc: @elastic/kibana-management in case you have any suggestions. Also, should we specify in which release we plan to add a fix?
