Describe the feature:

We'd like to request a change for the Add remote cluster page when Kibana is part of a hosted environment, to help cloud users easily identify that they need to use Proxy mode when they want to add a remote cloud deployment and also indicate where they can find the needed values.

This is an update the already merged PR #61639 (issue: #61503)

These changes would be:

• Updating the existing callout to have a title and a description (title would nudge users to enable this for cloud deployments and the description would indicate where to find the needed values
• Invert the order of the input fields in this order:

1. Proxy address
2. Server name
3. Socket connection
