After the Disease enum has been modified in (#13609) as part of epic (#13608), the case data form needs to be updated with Cryptosporidiosis-specific details

SORMAS will support Cryptosporidiosis disease case surveillance

Acceptance Criteria

• 'Clinical measurements' section should be hidden
• Cryptosporidiosis-specific symptoms should be visible
  • Abdominal cramps - mapped to abdominalPain
  • Diarrhoea
  • Nausea
  • Vomiting
  • Weight loss: weightLoss - (new symptom)
  • If Weight loss set to yes opens a text field ‘How much weight did you lose (kg) - weight_loss_observed (new Symptom)
  • Dehydration
  • Fever
  • Fatigue
  • Loss of appetite - mapped to anorexiaAppetiteLoss
  • Other: if yes opens a free text field
  • First symptom is hidden.
  • First symptom is hidden.
  • New field 'symptom_persistence' with values Y/N/U should be added to capture the current condition of a person
  • If 'symptom_persistence' field set to YES,
    • A new calculated field called 'symptoms_duration' should be displayed.
  • Complication section displays the following symptoms.
    • Complication (New Symptom)
    • Reoccurrence (New Symptom)
    • Time off work or school (time_off new field)
      • If time_off is set to Y, a text field 'days_time_off' should appear to record the duration of the absence.
    • A new symptom as 'has_overnight_hospitalization' should display,
      • If the 'has_overnight_hospitalization' field set to Yes, Symptoms should be saved and the user should redirect to the hospitalization tab.
