Feature Description

After the Disease enum has been modified in (#13602) as part of epic (#13601), the case data form needs to be updated with Giardia-specific details.

Added Value/Benefit

SORMAS will support Giardia disease case surveillance

Acceptance Criteria

• If 'Currently Hospitalized' checkbox(Y/N/U) is set to Yes, DoA and DoD fields should be enabled.
• If DoD(Date of Discharge) field is not empty, a new calculated field like 'Length of stay' should be displayed with DoA+DoD field values.
• 'Reason for hospitalisation' field is mandatory.
