Replace remaining miden-base references with protocol across docs, READMEs, comments, and Cargo.toml repository URL. CHANGELOG.md is left unchanged as those are historical records.

Looks good to me! I'll merge this to unbreak the doc links in various places. The link to docs.miden.xyz is broken now, but it should no longer be with the next release, so I think that's fine.
