• BARANGAY CERTIFICATION that indicates affected status (e.g., disaster-affected, indigent, etc.)

• SUPPORTING DOCUMENTS:

   For example:

    ○ School certificate (for educational aid)
    ○ Hospital bill (for medical aid)
    ○ Photos of damage (for calamity aid)
