- [ ] Confirm healthcare power of attorney
- [ ] Locate or complete advance directives
- [ ] Create a medication list (with dosages and times)
- [ ] Get contact info for all doctors, pharmacies, and specialists

🏠 Environment & Safety
- [ ] Assess fall risks (rugs, stairs, lighting)
- [ ] Post emergency numbers visibly
- [ ] Ensure bathroom safety (grab bars, non-slip mats)
- [ ] Consider a medical alert system if mobility is limited

🧠 Emotional & Social Support
- [ ] Find at least one support group (in person or online)
- [ ] Identify 1–2 backup caregivers or helpers
- [ ] Make time for respite—schedule it like an appointment

💬 Communication
- [ ] Have “the talk” about care wishes (start small if needed)
- [ ] Keep a shared calendar for medical appointments
- [ ] Set up a group text or email to update family
