Thank you for your interest in establishing an account with CDA Paving and Concrete Specialties. In order to open an account for your company, we require the completion and return of our Credit Application. It is important that you fill out information for all three trade references. If you are doing business in the State of Washington, please include your Washington Reseller Permit.
Once you have completed the required information, please email the application to Credit@WeAreCDA.com, fax it to (208) 665-9236, or mail it to 3110 N. Government Way, Coeur d’Alene, ID 83815. If all the information is accurate, we will contact you with the terms and credit amount within ten business days.
