Agate Housing and Services offers a variety of options to help you find stable housing and a sustainable path ahead.
To find housing through Agate, you must be referred by your case manager through Coordinated Entry.
Scattered-site housing
Housing Support:
If you are a Hennepin County resident or transferring from a Housing Support facility in another county, you can be placed in apartments throughout the county. You must meet the state’s long-term homeless definition to qualify for funding. Landlords/property managers each set their own screening criteria for tenants, so a variety of housing histories and criminal backgrounds can be considered.
Our services include:
- In-home visits from case managers to promote stability and provide mediation as needed with landlords, neighbors, etc.
- Assistance with budgeting and shopping
- Referrals to local community, health and social resources

Families (including youth head of household)
If you are an unsheltered household, please contact the Hennepin County Emergency Shelter and Diversion Hotline at 612-204-8200. An advocate will help you navigate options to resolve your housing crisis. Once all options are explored, if eligible, you will be assisted with setting up an appointment for a coordinated entry assessment.
If you are staying at a non-Hennepin run shelter (Mary’s Place, Hope Street, etc.), please have your advocate / case manager fill out THIS online form and submit an online shelter verification form.
- This form is ONLY for families that include minor children (under 18), or people who are pregnant.

If you are currently in emergency shelter, request a case manager from the shelter you’re staying in and mention your interest in a CES assessment.
On-site permanent housing
Housing Support:
County-funded housing supports rooms for single adults who have experienced homelessness.
Housing can only be accessed through Hennepin County Coordinated Entry.
Priority is given to individuals living on the streets or in shelters with mental, chemical and/or physical health concerns, difficulty meeting basic needs, or are vulnerable to exploitation.
Our services include:
- Wraparound case management services to help clients clarify their goals and identify available resources and strategies
- Free laundry facilities
- Assistance with medications available
- Secure building with 24-hour staffing
- Social and community events
