  • If medical treatment is recommended and available, you can explore options in your state and schedule care. Treatment is not included in the Diagnostic Evaluation and is available for an additional fee.

The Diagnostic Evaluation does not include treatment. If clinically appropriate, you may schedule medical care, psychotherapy, or mental wellness coaching based on your needs. Services are billed separately, and you pay only for the care you choose.

1 Costs based on annual ongoing appointments for medical treatment. ADHD Online costs include five visits in a 12-month period. Traditional in-person treatment costs are based on nationwide averages from the December 2024 survey published by ADDitude. Actual prices may vary by location and provider availability. Costs do not include medication.

Insurance and Payment Options

Insurance

While we do not accept or directly bill insurance, many insurance plans can reimburse up to 100% of our services.We provide patients with itemized statements, also known as superbills, which include the necessary billing codes to file a claim with your insurance company. Some insurance providers may require you to submit a letter of medical necessity for this expense, please

HSA and FSA

You can use your FSA or HSA accounts to pay for your Diagnostic Evaluation, treatment, and therapy. You might need to provide both the original payment receipt and the itemized receipt as supporting documentation. For Mental Wellness Coaching, check with your insurance provider to confirm coverage. If your insurance provider requires a letter of medical necessity for reimbursement, please contact us for assistance with the paperwork.

Frequently Asked Questions

The Diagnostic Evaluation is $199, which includes:

  • Comprehensive assessment reviewed by a licensed psychologist in your state
  • Clinical diagnosis (as warranted) for ADHD, depression, or anxiety
  • Personalized report from your psychologist

The Diagnostic Evaluation fee does not include treatment. Treatment services, as warranted by diagnosis, are billed separately.

Initial Appointment: $199
After receiving results from our completed assessment you and your clinician will discuss options and develop your personalized Medical Treatment plan together.

Check in on Progress: $175
30 days after initial appointment. You and your clinician will discuss your Medical Treatment plan progress and identify any necessary changes.

Schedule Follow-Ups: $175
Every 90 days for a routine follow up.  Appointments are scheduled every 90 days for safety and best practice.

Note:
Providers may require an earlier appointment for medication or dosing changes or any change in your medical condition.
Receiving three prescriptions within this 90 day period is not guaranteed and depends on various factors.

The Pay Plan is a flexible payment option that allows you to purchase our $199 Mentavi Mental Health Diagnostic Evaluation and divide the cost into three manageable payments of $73 each. This plan makes it easier to access the Diagnostic Evaluation without paying the full amount upfront.

We do not participate nor directly bill insurance companies. However, you may be able to get reimbursed from your health insurance company, possibly up to 100% of what they typically cover. To find out if you can get reimbursed, please call your insurance company. You can also send them an itemized receipt showing what services were provided and what you paid for them.  

If you have an HSA or FSA account for your health insurance, our services are qualified expenses, so you can use that money to pay for them.

We do not accept Care Credit health and wellness cards. 

Helpful Tip: Occasionally our card reader may not recognize the HSA/FSA card and payment may be declined.  There is no need to be alarmed!  Please contact one of our patient advocate support specialist by Live Chat on our website and they will be more than happy to assist you with the payment process.

We do not bill any insurance companies directly.  If you are a member of a Medicaid plan please check with your plan's coverage details to see if they will reimburse you for our assessment or any medical visits for the treatment and follow up if needed.  Not all of our providers are Medicaid certified, which may influence your ability to have services with us reimbursed, or have your prescriptions paid for by your Medicaid plan.  

If you choose to pay out of pocket for our assessment and diagnosis, you are able to take your results to another provider, like your primary care provider, who is Medicaid certified, and in your network. Then, if medication is prescribed, the medications would qualify for Medicaid coverage if available in your plan.

If you choose to pay out of pocket and see our providers for the treatment of your ADHD or related conditions we cannot guarantee that the medications would be covered by your Medicaid plan.  

Helpful Tip: Many of our patients have had good experiences working with RxOutreach.  RxOutreach is a mail order pharmacy whose mission is to provide access to affordable generic medications.  RxOutreach is a cash pay system and they do not bill insurance companies.  If you are interested in learning more about them, you can review their website at rxoutreach.org.  If you have specific questions please go to our website and start a Live Chat with one of our patient support specialists.

Curious about stimulant or non-stimulant approaches?

If you’re navigating ADHD or other mental health challenges, you have options. At Mentavi Health / ADHD Online, we assess what works for you. 
Some may start with non-medicated approaches like coaching or therapy.  In other cases, patients may explore medication treatment paths when warranted by diagnosis. For some diagnosed with ADHD, the treatment clinician may work with the patient to determine if stimulant approaches work best for them. Stimulant approaches are certain medications under medical treatment that are generally Class II controlled substances. Non-stimulant options are another approach that can work for many.  Medication approaches can be used in tandem with coaching or therapy. 
 
The key is starting with our diagnostic evaluation, which is reviewed by a qualified healthcare professional. Getting clarity on what you’re experiencing is the first step toward feeling more in control. From there, you and your treatment team can determine the best path—whether that’s coaching, therapy, medication, or a combination that works for your life.

Mentavi Health is conducting ADHD research and is accepting a limited number of participants. Participants in our clinical study will get an ADHD Assessment at no cost.
