CLCH provides community health services to more than four million people across 14 London boroughs and Hertfordshire and commissions continuing health care across West and North London. CLCH is a prospective advanced foundation trust.

Every day, our staff provide high-quality community healthcare in people's homes and local clinics, offering more than 105 clinical services, helping people to:

We support our patients at every stage of their lives, providing health visiting for new-born babies through to community nursing, physical rehabilitation and palliative care for people towards the end of their lives.
In line with our Trust Strategy 2025-30 – Healthy Neighbourhoods, Thriving Communities – and the Government’s 10 Year Health Plan, we are actively supporting the development of a flourishing neighbourhood health service.
We are committed to moving beyond traditional healthcare delivery to improve health outcomes for entire communities, reducing health inequalities, and addressing the wider factors that affect health. Achieving this requires working closely with communities, partner organisations, and healthcare professionals to provide more effective, preventive, and inclusive care.

In 2025, we were appointed to host the integrator arrangements in four of the place-based partnerships we deliver services in. These include Kensington & Chelsea and Westminster (with GP federations and the voluntary sector taking forward some enabling functions), Hammersmith & Fulham, Harrow and Barnet (in partnership with the Barnet GP Federation).
In this role, we bring together partners across health, social care and the voluntary and community sector to design and coordinate neighbourhood based, proactive, and connected care.

The values were developed by staff, for staff, through a series of staff engagement sessions, and reflect how we work together, collaborate with our partners, and deliver care to the communities we serve.
