Doctors’ livelihoods can be determined by their rotas. We all knew the hours would be long, but what we didn’t expect was the constant battling over when those hours would be, forcing us into a continuously bad routine. This lack of day-to-day stability is further compounded by the fact that it is very typical for resident doctors to move up and down the country as part of their training, yet another step in removing predictability entirely. So-called ‘rota patterns’ invariably result in disparity in hours worked between colleagues doing the exact same job, for the exact same pay. These concepts can be difficult to explain to our colleagues outside of healthcare, who rely on software such as MS Excel for its true purpose (anything but making rotas for doctors). Preference based rostering is a concept that I came across recently. It stipulates that instead of forcing staff into a rigid, rolling template, doctors can input their requirements and shift preferences before the schedule is generated. Why shouldn't Dr. Clarke be able to reliably play his Wednesday evening football match, and why shouldn't Dr. Evans get to make it to her child's school play? Both are just normal life events, and with a flexible system, both can be accommodated. Obviously, there are caveats. The wards still need safe staffing at all times. The nights, the bank holidays, and the heavy on-calls aren't going anywhere, and we all still have to do our fair share of them. But preference-based rostering recognises that the total hours don't need to change for our quality of life to improve. It simply replaces blind allocation with mutual flexibility, allowing a team to shuffle the exact same hours into a schedule that actually works better for everyone. If this initiative can move us inches closer to the respect that resident doctors deserve, I see that as a big win. What are your thoughts?
