Great News Nebraska Opens a New Alternative Pathway for International Medical Graduates(IMGs would be able to practice without residency ) Nebraska has officially enacted Legislative Bill 1212, creating a new pathway for eligible International Medical Graduates (IMGs) to practice medicine without completing a traditional U.S. residency. While the legislation is now law, it will take effect on August 1, 2027, once implementation regulations are finalized. Key Eligibility Requirements Applicants will generally need to: - Graduate from an ECFMG-recognized international medical school. - Hold ECFMG Certification. - Pass USMLE Step 1 and Step 2. - Complete postgraduate training substantially equivalent to a U.S. residency. - Have at least three years of recent independent practice (or equivalent training accepted by the Board). - Hold a current medical license in good standing. - Be legally authorized to work in the United States. - Obtain employment with an approved participating healthcare entity. USMLE Step 3 is not required for the initial provisional license. The Licensing Pathway Provisional License (up to 3 years, renewable once): - Supervised clinical practice - Competency-based assessments - Individualised learning plan Transitional License: After successful evaluations and passing USMLE Step 3, physicians may practice independently in designated physician shortage areas. Full License: Following successful completion of the required provisional and transitional practice periods, physicians become eligible for a full, unrestricted Nebraska medical license. Key Takeaways - Start preparing your ECFMG certification and USMLE Steps 1 & 2 early. - Keep detailed documentation of your postgraduate training and clinical experience. - Maintain a clean, unrestricted medical license. - Begin connecting with Nebraska hospitals and participating healthcare organizations before the pathway becomes operational. Nebraska joins a growing number of states developing alternative licensure pathways for experienced international physicians while addressing physician shortages. Although applications are expected to open in 2027, eligible IMGs can begin preparing now.
