Jack W. Hoffman
September 26, 2005 – January 15, 2025
Jack Hoffman was the inspiration behind the foundation. After being diagnosed with brain cancer at age five, Jack won the hearts of many with his touchdown run in the 2013 Nebraska spring game. His legacy will forever live on through the work of the Team Jack Foundation.

Power5 Pediatric Brain Tumor Initiative
Team Jack has pledged $5 million to fund the Pediatric Brain Tumor Program at the University of Nebraska Medical Center Omaha and Children’s Hospital Omaha.
