He was referring to Tina's new tits. Women sometimes refer to their breasts as 'the girls'.

Well I have heard that once or twice, but I would never expect a doctor, who is supposed to be professional in these matters, to say something like that lol. I mean if my friend got that surgery while I was there and the doctor came up to me and said that I would have been like, "I think you have the wrong person. I didn't come here with a pregnant person, my friend came here for breast surgery." XD
