Little is known of the origins of the Domains of Dread, but what’s certain is that they began here. Strahd’s villainy, his connection to his land, and his entanglement with the sinister priests of Osybus, drew all of Barovia into the Shadowfell. This began a succession of torments that haunt the Count still and spill forth to drag others into the Land of the Mists.

Strahd is a patient and dramatic mastermind. His statistics are similar to those of a vampire and his spellcasting prowess is formidable, enabling him to face most threats directly. Strahd ever seeks to escape his boredom and draws challenges out for as long as it entertains him. However, if insulted, the Darklord can turn all of Barovia against his foes.

One of D&Ds' most famous and popular BBEGs, Strahd is THE Vampire in this universe. The little blurb above doesn't begin to cover the tragic (mostly self-induced) back story of Strahd. R–Regis

Regis, jokingly called Rumblebelly by Bruenor Battlehammer, was the halfling member of the adventuring group known as the Companions of the Hall. Regis preferred to live in comfort more than anything, even more so than his friends. He also hated violence and cruelty, and avoided it wherever possible. When forced into a fight, he would assist his friends and make only sneaky strikes against their foes. He could be very cunning and subtle and his experiences and adventurers led him to be wise and observant.

You knew he was coming. Yet another of Drizzt's companions. Regis is a Halfling rogue. A little chubby and played somewhat for laughs, he is still a stalwart member of the team and can be counted on when the chips are down. Four in-progress pictures below.

Q–Queen Neri

Queen of the Storm Giants and wife of King Hekaton. She held a fondness for the small folk. She visited them often in the years before the Ordning was sundered, rising up out of the sea to meet them on the shores of the Sword Coast. Neri continually urged her husband to respect the civilizations of the small folk and leave them alone, if he could not countenance forming alliances with them. Hekaton, inherently distrustful of the small folk, wanted nothing to do with them, but he respected his wife's desire to treat them on occasion.

I wouldn't call Queen Neri an NPC per se. She is more of a catalyst for the events in the Storm King's Thunder campaign. Her fate starts a chain of events that cause gigantic trouble for your PCs. Six in-progress pictures below.

P–Pincushion

Not to be mistaken from the Carrionette from Van Richten's Guide To Ravenloft. Nevertheless, this tiny construct can be mistaken for an inanimate doll. What lies within, though, is a mean spirit that takes delight in misbehaving. The Living Doll can regenerate its hit points as long as it does not take any Psychic or Fire damage. The Living Doll also has a Cackle ability that forces creatures within 30 ft. of it to laugh uncontrollably. Creepy.

There are a lot of scary monsters in D&D, but is there anything scarier than an evil living doll??? Pincushion is a Living Doll construct/NPC from The Wild Beyond The Whitchlight campaign designed to torment/aid PCs in their quest. Five in-progress pictures below.

O–Orond Gralhund

Orond Gralhund was a noble patriarch of House Gralhund in Waterdeep in the early 1490s DR. When in 1491 DR many members of the Lords of Waterdeep were killed in a nefarious plot, Orond expected that his wife would fill one of the vacancies but that never happened, despite all the bribes and promises. Orond became deeply angry about this.

The second drawing of an NPC from the Waterdeep: Dragon Heist campaign. A cowardly nobleman, Orond helps set events into play. He and his wife's ambitions are the catalyst for much of the plot. Six in-progress pictures below.
