Appoint a Proxy

This form will generate a completed Proxy Appointment (Form 1) under the Strata Schemes Management Act 2015.

A proxy allows another person to attend and vote at a meeting on your behalf. You will receive a copy of the completed proxy form via email for you to download and submit.
