7 easy steps
Uses: Selective Color, Levels, Saturation

1. First, resize or crop your base layer to 100 x 100 pixels, because it is the actual size you need for the icon.

It should be like this:

You can choose whether to sharpen or blur etc. your icon base, its up to you :) I sharpened my icon.


2. Duplicate the base layer. Set the duplicated base layer to Screen. Now set the layer's opacity at 48%

It should look like this:

3. Create a new layer, and fill the layer with #346398. Set the layer at Exclusion and the opacity at 26%.

The icon should look like this:

4. Go Layer > New Adjustment Layer and create a Selective Colour Layer with the following settings:


Reds:
Cyan -36
Magenta 0
Yellow +28
Black 0


Neutrals:
Cyan +25
Magenta +3
Yellow -13
Black -33




The layer should now look something like this:

5. Go Layer > New Adjustment Layer and create a Levels layer with the following settings:

RGB:
Input levels: 0 , 1.14 , 246
Output levels: 0 , 255



Should look like this:

6. Do another Selective Colour layer again - Layer > New Adjustment Layer and create a Selective Colour Layer with the following settings:

Reds:
Cyan -100
Magenta 0
Yellow +62
Black -23


Yellows:
Cyan -100
Magenta +77
Yellow +100
Black 0


Neutrals:
Cyan +100
Magenta +16
Yellow -34
Black -12

7. Lastly, go Layer > New Adjustment Layer and create a Hue/Saturation Layer with the following settings:

Master:
Hue: 0
Saturation: +12
Lightness: 0



And now it should look like this..
