mailform with attachment

Hi, I'm new to the community and have some PHP skills.

I am working on a project where a user comes to my website and fills out a form then the form contents are emailed to me along with an attachment (CSV) containing the users form field input.

Does anybody know of an open source code I could use for this project, or is it something I will have to build myself?
