Book recommendations

I'm rapidly coming to the conclusion that I need a new book on PHP. With that in mind I'm soliciting opinions on which one I should look for. Here's me: good with HTML and CSS, but have a hard time getting through scripting languages including Javascript, Perl and PHP. What I am looking for is a book that covers PHP and using it with MySQL, and includes examples of what you can do with the two of them. If one of these examples were of a page for displaying different items in a database, based on whatever criteria you select, and also had some means for adding, modifying and deleting what was in the database, that would be excellent. So what books do you love and would recommend to someone who's still pretty new at all of this and doesn't have the easiest time grasping the concepts of this sort of scripting? Thanks.
