KU Wichita Student Health Services Patient Information and Health History Form

Dr. Newell will send you the PDF form this summer. This form must be printed, completed legibly in its entirety, then signed and dated and brought to Wichita campus orientation on Thursday, August 20, 2026. This form will be provided to student health services to prepare your electronic health record prior to your student health onboarding session in September or October.

P1 Pharmacy Student Onboarding at KU Wichita Student Health Services

P1 Pharmacy Student Onboarding days are select dates and appointment times in which the medical director of KU Wichita Student Health Services and the School of Pharmacy Wichita Campus Immunization Coordinator are made accessible to pharmacy students only, thus providing pharmacy students a unique opportunity to have sole access to these providers to get established with student health services to receive future primary care services if and when needed and begin the immunization compliance process.

P1 Pharmacy Student Onboarding dates for the fall 2026 semester are the afternoons of Wednesday, September 9th, Wednesday, September 23rd, Wednesday, October 14th and Wednesday, October 28th. Onboarding appointments are in 30-minute increments beginning at 2:00 p.m. All incoming Wichita campus P1 students are required to schedule an onboarding appointment to initiate the immunization compliance process.

Booking Your Appointment

Prior to Wichita Campus Orientation on August 20, 2026, please log in with your KU ID to complete the KU Pharmacy Booking Form. You will first need to provide your KU online ID on the Microsoft login page. You will then be taken to the KU log in page where you will enter your KU online ID and password. On the form, select a date (Sept. 9, Sept. 23, Oct. 14, Oct. 28) to see available times. Then complete and submit the form to book your appointment. Appointments are on a first-come, first-served basis. 

Greater Wichita YMCA Student Recreation Membership Application

The Greater Wichita YMCA Student Recreation Membership Application and waiver must be printed, completed in its entirety, and brought to Wichita campus orientation on Thursday, August 20, 2026. This form will be provided to YMCA personnel to activate your annual student recreation membership.

KU School of Medicine-Wichita Wellness Room Exercise Readiness Self-Assessment. The KU School of Medicine-Wichita Wellness Room Exercise Readiness Self-Assessment must be read, printed, signed and dated, then brought to Wichita campus orientation on Thursday, August 20, 2026. This form must be completed and maintained on file with KU School of Medicine-Wichita Human Resources before you can be granted access to the campus Wellness (fitness) room.

Parking

Please refer to the KU School of Medicine-Wichita Campus Map and park in the gray permit parking lot located west of the westernmost building and enter through the west entrance (entrance R). You will not require a permit to park on campus the day of Wichita campus orientation on Thursday, August 20, 2026. You will be submitting information for your electronic parking passes during the Wichita campus orientation session.
