If you are looking to sign up for a Library Card with the Mid-Hudson Library System, fill in the fields below.

The Library Card record created will be reviewed for eligibility and duplication. If you need anything else, just give us a call or send us an email.

By filling out this form and following the instructions on the page that follows, you will be given a temporary library card number.

To complete your registration and receive your permanent library card, you must stop by the library in person and present photo ID and proof of your current address.
