This tutorial is for psp8 but i'm sure it's translatable for photoshop users.
So we're going to find out how i made this icon:
The starting image was this, and the lady is Mischa Barton.

1.) First off open up your image and go to window/duplicate because you will need the image twice.
2.) In one window crop a piece of the image that you want and resize it to 44x90. Then Adjust/brightness and contrast/automatic contrast enhancement and the settings i applied, Bias=darker Strength = normal and appearance = bold. Then adjust/sharpness/sharpen.
3.) Now go into the other window crop another piece of the picture that you would like to use. I resized this bit to 25x100 and then applied exactly the same settings that i just did to the other bit.
so we know have these 2 images ...
44x90
and 25x100
. Leave these alone for now.
3.) File/new and 100x100. Flood fill it with #ffffff (white).
4.) Now copy and paste both of your cropped items and place them as new layers above the white 100x100 background. I placed mine like this ...

5.) make a new layer. and using your select tool
select a rectangle like this
Flood fill it with a color of your choice. I chose a color from Mischas face (#FDEEED). You'll be going back to this later but just leave it for now.
6.) new layer. Flood fill with #001756 leave it at 100% opacity but change the blend mode to exclusion. It's looking like this now
7.) Now i'm going to use another of
disappearicons wonderful creations. Open this
up and paste it as a new layer above the exclusion layer. I set the blend mode to 'saturation (L)' and left it 100%. This gives me ...

8.) Time to add text. It's text on it's side so it'll be easier to roate the image before i apply it. Image/rotate/rotate clockwise 90. I use the font georgia in size 6, bold and with 2 spaces between each letter. The color of the text i took from mischas hat (#494756).
9.) Rotate the image back ... image/rotate/rotate counter clockwise 90.
10.) we're basically done now but for my final touch i selected my rectangle layer on my layers pallete and ... effects/3D effects/drop shadow and applied these settings ...

my layers look like this

and my icon is complete
