Note: This tutorial is more advanced, but still do-able if you know your tools. I have no clue if this is PS transferrable, seeing as I have no knowledge of PS, however, if someone wants to try, go right ahead.
Part One: Coloring
1. I started with an image I capped myself from The OC Season One DVDs, episode: "The Escape. Then cropped it to 100x100

2. The image was dark so I fixed it.
a. Duplicate the base.
b. Go to Image >> Adjust >> Brightness/Contrast (or Shift + B) and set both brightness and contrast to 40. Press ok. The image is now brighter:

3. Set the layer blend mode to soft light:

This technique I use a lot, it's caled a blue exclusion layer:
4. First make a new blank layer.
5. Fill it with this dark blue:
#12132C
6. Set layer blend mode to exclusion. The image I have now looks like this:

7. Now duplicate your base layer and drag it to the top of the layer palette.
8. Desaturate the image by going to Adjust >> Colors >> Hue/Saturation/Lightness (or Shift + H). The image should now be black and white.
9. Set the blend mode to soft light:

Now at this point the icon turns into something more about personal prefernce (brushes, gradients, textures, etc) but I'll take you through the steps I did.
10. I used this gradient on a new layer:

11. I set the blend mode to color and the opacity to 50%. Now it's like this:

12. For some texture I used a brush by
teh_indy (she's great at simple but elegant brushes):

on a new layer in white.
13. Set the layer blend mode to dodge. So now my icon looks like this:

Ok now the coloring is done, the layer palette should currently look something like this:

Part Two: Ripped Paper Effect
1. Keep your half finished icon open still but make a new blank transparent background file (Ctrl + N).
2. On the new image, use this brush from
teh_indy in size 100:

I used the same gradient color as I used earlier to position the brush at the bottom of the icon until I liked it:

3. Then I went to Image >> Flip and then Image >> Mirror. Now the ripped paper was at the top and bottom:

4. Merge all the layers together by going to Layers >> Merge >> Merge Visible or by right clicking on a layer and going to Merge >> Merge Visible.
5. Copy the image and go back to your half finished icon. Edit >> Paste >> As New Layer (or Ctrl + L). The paper should now be on your image:

6. Alright, make a new layer and set opacity to 76%.
7. Again a brush by
teh_indy

At size 50, in a light gray (#C0C0C0), place the tape overlapping both the paper and the picture at the bottom:

8. I didn't like the position so I rotated it by going to Image >> Rotate (Ctrl + R) with settings like this:

Afterwards the tape looked more naturally placed on the icon:

9. New layer. 76% opacity.
10. Using the same settings for the brush above, place another piece of tape at the top of the icon, overlapping the picture and paper again.

11. This time I rotated the tape 7 degrees right for this:

Your palette:

Part Three: Text
This part is so short it shouldn't be it's own part but I didn't want to add it with the paper.
1. New layer.
2. With the eyedropper tool choose a color from your icon for your text.
3. Then:

The "Tijuana" has spaces between it: T i j u a n a --- like so.
4. Now go again to Image >> Rotate and do 5 degrees left. And you now have your finished icon:

5. Save as .png for the clearest image and enjoy.
Please comment with questions/constructive criticism or icons you've made with this tutorial. I hope this works out.
