Made in Photoshop CS2, but I think it will work in other versions of Photoshop. We will be using Color Balance, Curves, Selective Color, Hue/Saturation and Color Fill layer.

STEP ONE
Open your photo and make anything, which is necessery to make clear base. I opened Heechul photo, which was taken, when he was practicing to musical "Xanadu": http://i37.tinypic.com/2ykb42o.jpg I cropped it and resized to 100x100. Now I have ready base:

STEP TWO
Duplicate you base and set to Screen 50%.

Make a Curves layer.
RGB
First point:
input: 7
output: 15
Second point:
input: 50
output: 70
Third point:
input: 150
output: 180
RED
First point:
input: 80
output: 90
Second point:
input: 165
output: 178
GREEN
First point:
input: 50
output: 60
Second point:
input: 130
output: 150
BLUE
Firs point:
input: 44
output: 54
Second point:
input: 150
output: 200

It looks nice, ne? You can stop here and add text, brushes, etc. if you want, but I will go on. So let's continue.
STEP FOUR
Make a Selective Color layer.
REDS: +54, +30, -10, +10
YELLOWS: +40, +30, +10, -10
WHITES: +10, +10, +10, -40
NEUTRALS: +10, +5, -10, +20
Make sure that Relative is selected.

STEP FIVE
Make a Color Fill layer. I filled mine with #060E1C. Set it to Exclusion 100%.

STEP SIX
Make a Hue/Saturation layer.
MASTER: 0, +20, +1
REDS: +10, +10, +10
YELLOWS: +10, +10, +10
GREENS: 0, +10, +10
CYANS: +10, +10, +10
BLUES: +10, +10, +10
MAGENTAS: +10, +10, -10

STEP SEVEN
Make a Color Balance layer.
MIDTONES: -1, +11, +27
SHADOWS: -10, +10, +10
HIGHLIGHTS: +9, +10, -3
Make sure that Preserve Luminosty is selected.

We can stop at this place and leave icon, as it is now. You can also add some brushes, text, whatever you want. I added text "intense" with font "Footlight MT Light" and I ended up with this icon:

Easy tutorial, don't you think so? ^^.
You can of course play with settings, add some new layers etc.
