Request #3 : BL Mini-Movie Avatar.

Hi again!  So Roby, a girl I know from FF made this avatar:and I think it is AMAZING.  I've pmed her but she hasn't written a tutorial for it, just learned from other tutorials...  So I was wondering if anyone knew how to make something similar to this?
I use Photoshop and Imageready.  Thanks in advance!
