Made in Photoshop CS3

00. Prepare your base however you like. (e.g. Sharpen, crop, etc.)

01. Make a new Curves layer. (Layer>New Adjustment Layer>Curves)

Second Point: Input = 196; Output = 180

02. Create a Hue/Saturation Layer. (Layer>New Adjustment Layer>Hue/Saturation)
Put in these numbers...

Saturation: +23

03. Time for Selective Coloring! (Layer>New Adjustment Layer>Selective Coloring)

REDS: -100, -36, +100, 0
YELLOWS: -100, +27, -40, +4
CYANS: -24, -11, 0, -10
NEUTRALS: +22, -27, +11, 0

04. Make a Photo Filter Layer. (Layer>New Adjustment Layer> Photo Filter)

In the dropbox, select 'Cooling Filter (80)'.
Make sure the 'Preserve Luminosity' is checked.
Leave everything else alone.

05. Create a Channel Mixer Layer. (Layer>New Adjustment Layer> Channel Mixer)
Input these settings.

RED: +105, +16, -24

06. Make a Gradient Map Layer. (Layer>New Adjustment Layer> Gradient Layer)

Choose the gradient going from black to white. If it isn't, select 'Reverse' under 'Gradient Options'.
It should be the third one from the left, first row.

SET THIS LAYER TO LUMINOSITY 63%.

!Note: If it comes out funky looking, then refer back to the part about 'Reverse' in the Gradient Options.

07. Create a new Exposure Layer. (Layer>New Adjustment Layer> Exposure)
Input these settings.

EXPOSURE: +0.10
OFFSET: -0.0098
GAMMA SELECTION: 1.00

08. Make a Fill Layer with #231609. (Layer>New Fill Layer>Solid Coloring)
SET THIS LAYER TO DIFFERENCE 30%.

09. Make another Fill Layer, this time with #ffcf9f.
SET THIS LAYER TO COLOR BURN AT 14%.

10. Make the last Fill Layer with #091f07.
SET THIS LAYER TO EXCLUSION AT 63%.

AND YOU'RE DONE!! I'd like to see your results. :)
I prefer comments to be made here. But you don't have to.
