This tutorial is going to be pretty straight up as this type of icon is really easy to create. It will be short, basic and to the point, which means every detail will not be explained. I will not be providing a .PSD for this tut because this is an easy effect to achieve.
YOUR IMAGE/CROPPING
a) Started off with my screencap, it's a screenshot from House 1x18 Role Model from the scene where Cameron is at House's apartment and she's telling him she's going to quit. One of my favorite eps and one of my favorite scenes.
b) Pasted my screencap (without resizing) into a 200x200 square and using my free transform tool, scaled down the image and cropped it.
c) Resized to 100x100.
COLORING/ADDITIONAL EFFECTS
a) Added a Curves adjustment layer to brighten the icon.
b) Added a color balance adjustment layer. MIDTONES: 0, -15, 0
c) Added a Hue/Saturation adjustment layer.
d) Opened a new Curves adjustment layer, left the settings as is, hit ok and set the layer to Soft Light.
e) This made the icon too red, so I added another Color Balance Adjustment Layer. MIDTONES: -24, 0, 0
f) Added a Selective Color Adjustment layer.
g) The icon wasn't bright enough, so I duplicated my base, dragged it to the top and set it to Screen at 37%.
h) Flattened my layers and then duplicated the new base. Selected the layer below the duplicate (should be labeled background) and deleted it.
i) Pasted this texture on top of my layers and set it to Multiply. I'm not sure who made this particular texture. If it's you, let me know and I'll credit.
j) Selected the layer below the texture, and using my free transform tool, scaled the icon down a little so it fit inside the texture better.
k) Copy-Merged my layers (select all --> edit --> copy-merged --> edit --> paste).
l) It still needed a little brightening, so I added a Gradiant Fill layer. Decreased the opacity of the layer like so: OPACITY: 25% FILL: 75%
m) Copy-Merged again and set the layer to Soft Light.
n) Lastly, I added a tiny text brush on the far right corner of the icon.
o) Flattened my layers and saved the finished icon.

Not really that close to the original, I know, but basically this was how I achieved the effect! :) this icon was also asked about and basically the same steps were used to achieve the effect with the exception that the icon is black and white. Frame textures are some of my favorite textures to use and I do use them quite frequently on an icon -- especially if the scene and/or character I'm iconning is difficult to crop.
