First tutorial...
This is mainly for 
vinyldaiquiri, but its also for everybody else.^^
The question was about icons like this one... (post here)
...so I decided to help since I had just been showing someone the same thing the day before.
This if for Adobe Photoshop
Start out with a simple 100x100 background (white color). Make sure that you have your image open in another window in photoshop so in case you mess up something, you don't have to start over. I chose this image.
Selet the Hiroshi picture before clicking on Image, Canvas Size. Make sure the drop down boxes are on "pixel" before changing numbers. I first took the smallest number of the two (206, the width) and changed the image to a 206x206. I fiddled with the numbers for a while before coming up with this..

I kept it large while erasing the background...
It will look a little bad, but once you resize it...
...it isn't as bad. (Image, Image Size, 100x100)
Back to the white background.
Make sure that it is 100x100 and the resolution is 72! The resolution is important for the fonts!
One the layer right above the white background, put your text. I put it in size 6, font Minion.
Add another layer. Click on paint brush and set the opacity to around 70% before chosing a color. I chose gray (the first row at the end in the normal colors). I then sharpened the text layer.
Next I added the picture in, dragging one image to the other.
After that, you can either leave the picture how it is, or take it one step further and add more text in on the top. Or another effect^^ Playing with colors and tints are also good.
I added more text to mine, making the beginning picture.
When you're done, the layers should look something like this...

EDIT: I don't have the original 1.2.3. thing, but I attempted to get the images as close as possible to how they were!
