Making icons Crispy

When I make icons, for some reason I am unable to get the image to look sharp. This usually happens when I try to make icons out of quicktime files though, like small videos, not with DVD screencaps. Can someone offer their tips on how to smooth out the pixels in the picture, heighten the contrast, and keep the image sharp and crisp?
