1. The first thing you need to do is locate the picture you want to use and resize it to 100x100. Now the picture is still a bit dark, so I brightened up a bit using brightness/contrast
2. Now we are going to duplicate the image twice and set both of the new layers on screen. Then we will add some Gaussian Blur to the top layer, resulting something like this:

3. Next step is adjusting the color. So we add a New Adjustment Layer-Color Balance and put the following:

4. Now add a New Adjustment Layer-Hue/Saturation and take the saturation levels up between 40 and 45...I used 44. You should have something like this:

5. We now add a New Adjustment Layer-Color Fill-#8AD0E2 and set that layer to Color Burn. Then we add another New Adjustment Layer-Color Fill-#ECCE9E, and set the layer to Multiply. You get this:

6. Almost done...now all you need is add some texture to your icon. Use this texture:

once you add it, set it to Soft Light.
7. To finish up, just add some text to it and there you have it:
