Note: I think it's not translatable. I used Adobe Photoshop CS2. Could possibly be done with the older versions.

1. Prepare your image. Crop, sharpen, soften, etc...
2. Duplicate your image.
3. Desaturate the second layer.
4. Set the desaturated image to Overlay 100%.
5. Duplicate the desaturated layer and set that to Soft Light 100%.
6. Go to Layer > New Adjustment Layer > Color Balance. Set your settings to the following :

6. Go to Layer > New Adjustment Layer > Color Balance. Set your settings to the following :
Note About Step #6: If you're not satisfied with the result, you can play with the settings.
Optional : If the image is a bit dark, go to Layers > New Adjustment Layer > Brightness and Contrast and play with the settings until you get the result you want.
7. Add brushes, text or whatever you want. You can use the blur tool to soften the skin and the sharpen tool to sharpen the lips and the eyes.
8. Flatten your image.
