Help meee!

How do I do get the effect in icon in in PS7? (the horizontal lines effect):

I am able to do transparency boxes, such as this: and recently started playing around with text and the "stroke" feature to make this icon:

Can someone illuminate me...please ;_;? (Also, it would be nifty if someone could tell me the name of the font used in the first icon ^_^)
