1. Crop your base, sharpen, etc. This happens to be a screencap from the video for "Basket Case" by Green Day.

2. Create a new layer and fill it with #000000, then set it to Hue and change the opacity depending on your image. This mutes the colors of the base slightly and makes it easier to work with.

3. Add another new layer, fill it with #000050 and set it to Exclusion, 100%. This gives the icon that "vintage" look that was popular before Selective Color ruined everything. [I'm very anti-selective color. I don't understand what people like about it. Anyway.]

4. Duplicate the layer you made in the previous step and lower its opacity. This increases the intensity of the "vintage" look.

5. Create yet another new layer and fill it with #AFE7FF and set it to Color Burn, somewhere between 75 and 100%. This darkens the colors slightly while also giving the icon a slight blueish tint.

6. Paste the scratch texture onto a new layer and set it to Screen. This removes all the black, leaving just the scratches.

7. Duplicate your base layer, drag it to the top of your layer palette, desaturate it (Ctrl + Shift + U in Photoshop 7, not sure about any other program) and finally set it to Soft Light. This mutes out the colors even further while enhancing the "outlines" of the subject.

8. Finally, add a 1px border to your icon in #000000 and set it to Soft Light. This gives the icon a finished look, but without the harshness of a plain black border.
Your layer palette should look something like this:
