Made in PSP 7, but translatable :)
Super easy, too!
Right, first things first, crop and resize your pic to icon size :)
Duplicate the bottom layer, and set to Overlay.
Make a new layer, and fill with #CAA45C. Set the layer to Darken
Make another new layer, and fill with #FF0000. Set the layer to lighten, and adjust opacity to 80
Add this border...
Add text, and Ta-Dah!
Icon :)
PS. For text I used Sprint SF at 16pt, coloured white (#FFFFFF) Set the layer to Lighten :) (At least, I think that's what I did...)
