I used Photoshop CS2, but I'm sure it can be translated. No Selective Colour/Curves.
Warning: Image Heavy.

crop your base.
duplicate, set to screen 60%.
duplicate the base again, bring to the top. set to soft light, 100%.

colour burn, 100%.

multiply, 40%.

multiply, 50%

duplicate your base again.
set to overlay, 100%.

multiply, 50%.
erase parts covering the skin. this gives it a really nice glow and contrast,
without needing curves or brightness/contrast layers.

new layer, paste with scanlines.
overlay, 40%.
erase parts covering face.

oo8.
you may not need this, but i didn't like the bit in the corner.
so I covered it with black using a round, soft-edged brush.
normal, 70% opacity.
