Let's make this icon using Photoshop 7:

First, I start with my base from A Walk to Remember

Now, this base is really dark, because this part of the movie was the same, so, after I crop the picture to where I want it, I do an Auto-Contrast (ctrl + shift + L)

It's still too dark for my taste, so I duplicate the layer ( ctrl + shift + J ) and set the top layer to Soft Light. Then, I go down to the bottom layer, and adjust the brightness. ( Image > Adjustments > Brightness/Contrast ) I set the brightness to 60. Then I flatten the layer.

Now, I add the color. First thing I do, is greyscale the picture, to make it good and ready for color. ( Image > Mode > Grayscale)

Next, I go and change it back to RGB Mode. ( Image > Mode > RGB Mode) Then, I go to the Gradient tool. Instead of using a pre-made gradient. I bring up the Gradient Editor, and click on Randomize...I let the program choose what color to use. Create a new layer, and fill the entire layer with the Gradient. Set the layer to Overlay, 100% Opacity.

That's too bright...so I lower the opacity of the gradient layer to 50%.

Repeat the above step, with another gradient, and set it to Soft Light. 75%.

Now, flatten the image.
Create a new layer, and pick a texture of your choice. In this case, I'm using a texture I made a while ago. I do it in Black, Overlay, 100%.

Now, I add another texture. White, Overlay, 25%.

Now we're adding a border to this icon. Do it in Black, Normal, 100%.

Finally, The Text. I am listening to MTV in the background, and some song just said "Love is Pain" and I think it suits this icon/movie, so I use that quote.
I use the font Champignon; 30pt; Strong.

Create a new layer. Next, I use the Rectangle Marquee tool, and select the area around the text. I choose a color from the background, and fill the square with that color. Normal; 50%.

Then, you want to put a shadow on that box. So on the new layer, go to Blending Properties, and put a drop shadow.

Final step; Do a free form Transform on the text. ( ctrl + T ) Then, go ahead and go back to the text, and give it a drop shadow too. Ta-da. You have your icon.
