it's not the same as the picture she requested, I made a quick new one.
I use PS CS2.
From: 
(don't know if it's the same cropping) to:
1. crop your base to 100x100, & do what ever you do when you make a base.
2. I added a coloring by _chokeanddie at http://community.livejournal.com/_hellsbelles/11888.html
3. erase the backround of your icon (do it better than mine, I just made this quick) with the backround eraser tool:

&erase all the parts you don't want (I just erased everything except ashley) in the final icon.
4. pick your new backround (I picked this texture: http://img61.imageshack.us/img61/7771/carton550rw.jpg by http://www.ex-posed.com/carton/ resize it to 100x100 & make a new window with it (ctrl+n) set it to 100x100.
6. Go back to the ashley picture then hit ctrl+a (to mark the whole icon) then ctrl+c (to copy the icon).
7. Now return to the texture & hit ctrl+v (to paste ashley on to the icon).
