Old film feel icons

How do I get an image to look like this?



I know old movies have that sort of fading/brightening variation on them naturally, but was wondering how to do this simply in Photoshop/ImagereadyCS. I'm also a bit of a dummy with pasting transparent layers in CS, the sequencing makes little sense compared to that of Animation shop...
