Well I've been following this community for while, so I thought I'd take a crack the whole tutorial thing. I used my Adobe Photoshop 8.0 and it's pretty simple - I mean if I can do it, any one can! ;P

Alright.
I started with this image from Vogue's huge September issue --->

I fiddled around with the coloring to make her skin paler, and eyes greener, and ended up with this --->

Then I did the ever-popular exclusion layer. I used #0C1628 and got --->

I made two raster layers filled with color #EBBFBF. I set them both to soft light, the bottom at %100 opacity and the top to %50 --->

It was far too pink for my tastes though, so I duplicated the base, draged it to the top and desaturated it. I set that layer to soft light at %50 opacity --->

Next, I placed this gradiant (sorry, I don't know who it's by): Image hosted by Photobucket.com and set it to overlay at %50 opacity giving me this --->

Now, I wanted to draw all the attention to her face, so I used my gradiant tool to blacken the lower right corner, covering her hand. I used a layer mask to erase what ever was cover her face, but you could just the eraser tool --->

Then I added added some tiny text, and a brush (again, I don't know who it's by but if you do, just let me know) --->

The last step was adding simple, black border, using the stroke tool set to 1px. And voila --->
