Brush Query

I'm trying to make my own brushes and currently, I'm doing great with text brushes but what I just can't seem to do is those design brushes with like feathers and stuff. I realise how vague that sounds, make this'll clear it up. See this brush below? How do I do that design thing (not the box)? I use PSP but I also have PS, but definitely more comfortable with PSP.
