Done in PSP8, but I'm sure it's convertable.
I first took this picture of Sharon Den Adel and resized and cropped it to 100x100. I also sharpened the picture and sharpened her eyes and lips a little more. Duplicated the base for later use as well.

I took this texture from
colorfilter and set it to overlay at 100%
->
Used another texture from
colorfilter and set it at softlight 100%.
->
Used yet another texture by
colorfilter and set it this time to darken 100%.
->
Used this by
_synergy and set it darken 60%.
->
I cropped a small piece (20x19) from the duplicated base I had saved earlier and mirrored it. I desaturated it to -70 (shift+h) and pasted it into the little box.
->
I used this templete from Mint Tea Graphics and set it to overlay at 35%.
->
I took the magic wand and put it around the box. I colored the black to be more fitting to the image. While it was selected, I also went to effects -> 3d effects -> drop shadow. Vertical set to 3 and horizontal set to -3, opacity 70%, and blur to 5.00. I clicked ok and went back with the same settings, but this time set vertical 1 and horizontal -5. Repeated it again at vertical -2, horizontal 1.
->
I'm not a fan of the small image's color, so I used this gradient from
rough_draft___, selected the small image and used the copy tool, copying the gradient at 48% color.
->
I added some small text by
ingenu0us, screen 100%.
->
I chose to increase my saturation, but other than that, you're done!

If you have any questions, feel free to ask. I'd love to see what you come up with!
