Needing help with this

I e-mailed the person who made this, but they couldn't offer me any help on how to make it. They just "fiddled" around with photoshop. *sigh* Since I'm very new to the game of avatars, just started making them last Tues. I really need a tutorial on how to make this. I'm using paint shop pro 7, so any help is needed! Thanks in advance!!!
