Start off with a cap...

I crop a screencap from Cinderella Story into 100x100 size. I also sharpen the image.

I duplicate the icon 4 times with the blend mode all being on screen - but I change the top layer to softlight and then desaturate it lightly by going to >> >>Adjust>> >>Hue and Saturation>> >>Hue/Saturation/Lightness and have your settings like this: Hue: -180 ; Satuartion: 0 and Lightnes:: 0
My icon should be bright like this:

Now on the second layer (should be named copy 3) I add a new layer and fill it with this blue color #122B41 and set the blend mode to exclusion.
My cap is brown like this:
I now copy the background layer and paste it over Raster 1 ( the new layer that I had made ) and set the blend mode to softlight.

Now I click on the top layer >> add a new layer >> fill it in with this gradient
and I set the blend mode to lighten. I merge all layers.
Now I take a brush from here and stamp it on in white!

All done :).
