How to get this icon:

[For PS7, but easily transcribes.]
First, we start with this cropped image of the fabulous Diego Luna:

Sharpen the image, duplicate it, and set the top layer to screen at 100% opacity.

Add this base by
dearest [who is spectacular] and set it to overlay at 100% opacity.

Duplicate the original image again. Make it soft light at 100% opacity.

Stamp this brush by
dearest in #006CFF. [Actually, it was originally along the top; I rotated it to fill the empty space.] Set it to normal at 87% opacity.

Add another one of
dearest's bases, and set it to multiply at 100% opacity.

The final layer is for your text. Type in whatever you want -- I chose 'CHAROLASTRA' as you, uh, can tell. Add a 3px stroke in #0096FF, and make this stroke overlay at 46% opacity. Once you're done, reduce the opacity of the layer to 87%.
And again, here's the end result:
