I'm assuming that you have a basic grasp of PS and key terms
Start with your image. Crop to down to a perfect square but don't resize down to 100x100 yet. Make a new layer and rename it "lips." Set the layer blending to "multiply" and make your fill colour a pale pink. Zoom in and using a brush with the hardness set to "0%," colour over her lips. The end result should look like this:

(Make sure that it's over every other layer, including the gradient layer, which you'll see in a bit.)
Erase any excess and then go ahead and resize. Duplicate your base layer, set the layer blending to "overlay" and rename it "blur." Go up to filter->blur->gaussian blur. Set the radius to around 1.5 and hit ok. Go back to your base and go to filter>sharpen>sharpen. Duplicate it again and this time, name it "screen" and set the layer blending to "screen." (NOTE: You may have to play with the opacity of this layer, depending on how dark/light your original is. This time, it was set to 100%) Set your screen layer in between your base and overlay layer.
In between your screen and blur layer, make a new layer. Name it "gradient screen" and yep, you guessed it, set the layer setting to screen. Set your foreground colour to "#822757" and your background colour to "#2D245B" Grab your gradient tool, make sure it's set to "linear gradient" and drag diagonally from the upper right hand corner to the bottom left.
Like so:
Your gradient should look like this:
and your icon should look like this:
You can stop here, or you can go on. There's just three more steps to go!
Make a new layer, over every other layer, including the lips layer. Set the blending to "color burn," bring the opacity down to "50%," and rename it "burn." Flood fill with a light pinkish tan. (I used "#E1B7A0")
Add text where-ever, and however you want.
Finally, how to make that neat 1px border:
Make a new layer. Rename it "border." Pick the colour you want for your border [a very dark colour works best] and then hit ctrl+the base layer. (The actual layer in the layer menu, not the picture itself) Go to select>modify>border. Input the number 1 and hit ok. Now hit ctrl+shift+i to invert the selection and hit delete about 3-4 times. Ctrl+d to deselect and voila. Instant border.
