Using the smudge/blur tools correctly?!

Am a relative icon newbie and I use PS7.
I have been following tuts on using the blur/smudge tools to make images better and while I can see I am making a difference using these methods, I can't seem to achieve the results I see can be possible, even if I am using a good quality cap or photo to start with.
My question is, can anyone offer any advice and also do you use the b/w defaults or should you take the dropper and select an area of skin when smudging and blurring? I've tried both but as I say, am still not getting the desired results!
